The way I originally implemented diseases, they won't necessarily respect
"blocked" squares when looking for someone to infect.  They very simply
looked at everything within a certain radius, walls or no.  And simple
is good, because of the expense of doing LOS calculations or some such.

Probably best would be to have a disease-free square.
